Allow patients to leave a note when booking an appointment

When patients book an appointment online, they must indicate a "Motive" when they book their appointment. These motives make it easier for you to organize your time in your timetable, but they don't always provide enough information about why exactly a patient wants to visit.

That's why you can offer your patients to add a non-urgent comment, when making the appointment.

The option to leave a comments with an appointments is disabled by default.
